TAVARES, Fla. -- Blue Rhino, owned by Ferrell Gas based in Kansas, is the second largest propane distributor in the nation.
The Blue Rhino plant located in Tavares is licensed and inspected by the Florida Department of Agriculture and Consumer Services.
In November 2011, The Occupational Safety and Health Administration cited the plant for a serious violation.
According to details on OSHA's website the citation the plant was cited for "hand and portable tools and equipment", a possible cleaning violation.
The fine cost the company almost $2300.
An agency spokeswoman said the plant was last inspected July 19 and there were no violations.
The plant was also inspected in March 2012 also with no violations.
The plant runs two shifts and workers were on the second shift when the explosions occurred.
The Tavares Fire Chief says the cause of the explosions is still under investigation, but said it was likely due to equipment or human error.
