JACKSONVILLE, Fla. -- A 21-year-old woman is dead after an accident on I-295 Northbound and Town Center Parkway.

It happened around 1 a.m. Sunday. According to the accident report, a tire blew out on the car, causing the driver, Hau Huy Nguyen, 23, to hit the guardrail.

His passenger, Diane Cassidy, 21, was ejected from the car and killed.
