in our communities through their selfless volunteer efforts.
By sharing these extraordinary stories, we hope to inspire
others to follow their lead.

Purpose:
- To honor individuals for their extraordinary deeds
- To encourage and inspire others
Qualifications:
- The individual must be 18 years of age or older
- The individual must be a resident of the First Coast
- The individual cannot be compensated for his or her deeds or service
Nomination Process
- Nominations will be accepted throughout the year
- Nominations must be made on a 12 Who Care nomination form and sent to:

- Nominations must include a photograph of the nominee and two letters of recommendation from individuals other than family members.
